The following list of indications for hyperbaric oxygen therapy is accepted by the Undersea and Hyperbaric Medical Society (UHMS) and typically covered by insurance.

UHMS Indications
Air or gas embolism Enhancement of healing in selected problem wounds Delayed radiation injury (soft tissue and bony necrosis)
Carbon monoxide poisoning and CO poisoning complicated by cyanide poisoning Exceptional blood loss (anemia) Skin grafts and flaps (compromised)
Clostridal myositis and myonecrosis, (gas gangrene) Intracranial abscess, actinomycosis Thermal burns
Crush injury, compartment syndrome, and other acute traumatic ischemias Necrotizing soft tissue infections Actinomycosis
Decompression sickness Refractory osteomyelitis (Infected bone)

The following list of indications for hyperbaric oxygen therapy is accepted by the American College of Hyperbaric Medicine (ACHM) and typically covered by insurance.

ACHM Indications
Acute carbon monoxide intoxication Acute peripheral arterial insufficiency ulcers Diabetic wounds*
Decompression illness Preparation and preservation of compromised skin grafts Venous stasis ulcers - recommended only if venous surgery, local wounds care, leg elevation, counter pressure support, and skin grafting fails*
Gas embolism Chronic refractory osteomyelitis, unresponsive to conventional medical and surgical management Decubitus ulcers - with underlying osteomyelitis - a compromised skin flap, or an infected wound*
Gas gangrene Osteoradionecrosis as an adjunct to conventional treatment Arterial insufficiency ulcers - which persist after reconstructive surgery has restored large vessel function*
Acute traumatic peripheral ischemia (when loss of limb function, limb, or life is threatened) Soft tissue radionecrosis as an adjunct to conventional treatment Necrotic wounds secondary to brown recluse spider bite*
Crush injuries and suturing of severed limbs (as in the previous conditions, HBO therapy would be an adjunctive treatment, when loss of function, limb, or life is threatened Cyanide poisoning
* Not in all states
Meleney ulcers Actinomycosis, only as an adjunct to conventional therapy when the disease process is refractory to antibiotics and surgical treatment  

 


Alternative Indications
The following list of indications for hyperbaric oxygen therapy is considered by the UHMS as off-label or investigational and is not usually reimbursable by insurance companies. However, many insurance companies may cover the costs of the physician consultation as well as pre- and post-treatment diagnostic test.
